At our Immunization Clinic, we believe in the power of vaccines to protect individuals and communities from preventable diseases. We offer routine vaccinations from childhood to adulthood, ensuring that everyone has the necessary protection against infectious diseases. We offer our services Monday – Friday, 8:00am to 4:00pm. Call us at 660-747-6121 to schedule your appointment. Walk-ins are welcome!


We provide immunizations through the Vaccines for Children (VFC) program. To receive immunizations through this program, children must meet one of the following criteria:


  • Uninsured or Medicaid ineligible
  • American Indian or Alaskan native
  • Underinsured (have insurance that does not cover vaccines or that caps coverage for vaccines, after the maximum coverage has been reached)


We provide immunizations for adults through the 317-funding program for uninsured and underinsured adults. Not all adult vaccines are available through 317 programs. 


With our partnership with VaxCare, we can directly bill most major private insurance companies.

  • WHAT TO BRING TO APPOINTMENT

    Please arrive 15 minutes prior to your appointment to complete your registration process. An up-to-date immunization record must accompany all children 18 years of age and younger to receive immunizations. Insurance information if applicable. 


    If you are pregnant and need vaccines, please bring a signed doctor's order to your visit.

  • Vaccine Schedules

    We follow the recommended vaccine schedules provided by the Centers for Disease Control and Prevention (CDC) for children and adults. These schedules outline the vaccines recommended at different ages to provide optimal protection against vaccine-preventable diseases.

  • Records Request

    Need to access your immunization records? Our team can provide immunization records for both children and adults for school, work, travel, or personal use. To request a copy of your record, click here.  Release of Information. Please mail or fax 660-747-1294 your request, and please allow 2-3 business days for your request to be processed from the day we receive it. 

  • Exemptions

    Johnson County Community Health Services does not provide parental or religious exemptions currently. Please check the Missouri Department of Health Services website or click here

  • SCREENING SERVICES

    Many contagious diseases have been managed by childhood vaccination efforts, but several of them—including measles, mumps, and varicella (chickenpox)—have resurfaced across the nation as immunity levels decrease. To help individuals know if they are immune or require a booster for these preventable diseases, please call our office for pricing and to schedule an appointment.
