We have a credentialed and professionally trained team. This includes Nurses, Physical Therapists, and Occupational Therapists. Our team works closely with your physician and is ready to develop your personalized home health plan. This plan will provide the most compassionate care—right from the comfort of your own home. Our team is committed to improving your quality of life!

Insurance: We are contracted with Medicare, Medicaid, and many Medicare Advantage and private insurance providers, including Blue Cross Blue Shield, Tri-Care, Humana, and others.  Our Intake team consists of nurses and insurance coders to answer any questions. 

Our team is available 24/7 for our patients, physicians, and referrals. When you or a loved one are being discharged from the hospital or care facility, ask to be referred to Johnson County Community Health Services for your care needs.
