Community Health Home Visits

We believe that your time and convenience are important priorities. Our Community Health Home visit can be used to help serve patients that may not have transportation to physician offices, or are home bound.  This service can help provide convenience to patients, family caregivers, and physicians alike.

As a service to our JOHNSON COUNTY residents, our agency will provide visits to perform limited skilled nursing services. Clients will be eligible for home visits by referral from a physician, hospital or family member.  A physician must order the home care, as all care will be coordinated under the direction of the physician.
