3-501.16A2 West side refrigerator was running temps from 46°F to 49°F. Thermostat was turned down by staff and prepared PHF were discarded. Temperatures dropped 5 degrees while waiting and measured 42 degrees and continued to cool. CORRECTED.

6-501.11 1) Counter top has holes on surfaces. 2) Sink has rough caulking that is difficult to clean 3) Cabinets are missing front doors.
6-301.11 Kitchen did not have hand soap readily available. CORRECTED.

COMMENTS:
