Johnson County Community Health Services operates under County Health Center Statutes, Chapter 205, RSMo. under its elected Board of Trustees. We serve as the County Health Center and operate three divisions: MICH accredited Public Health: a Home Health care agency and Johnson County Hospice Care; both federally certified and state licensed. Additionally, our Home Health and Hospice provide care to both Johnson and Lafayette county residents in their homes.

           Johnson County Community Health Services strives to IMPROVE THE QUALITY OF LIFE of our clients, their families and the community by providing our services under the supervision of licensed professional nurses and physician orders. Our Home Health team promotes independence and recovery in the home setting while our Hospice care utilizes a multi-disciplinary team of professionals to provide quality end-of-life care. All our divisions work collaboratively with area community groups and providers to address a wide range of health care needs to eliminate health disparities and increase the number of healthy years that Johnson county residents live.
