When Johnson County Community Health Services and the Community Health Assessment Resource Team conducted the 2007 Johnson County Community Assessment we identified three top local disease risks: Chronic Obstructive Pulmonary Disease, Diabetes & Heart Disease.
Now we are trying to determine what efforts and resources in the county are already working toward these goals. By gathering information on existing resources we hope to identify gaps. By knowing where gaps exist, we will know where to target future wellness programs and policies in the county.

All births and deaths that occur in Johnson County are recorded at the health department. The health department is the local registrar for all births and deaths that occur in Johnson County. Certified copies of birth certificates for a person born in the state of Missouri after 1920 or a death in the state after 1980 can be obtained at the health department. More info . . .

Johnson County Community Health Services |
Johnson County Community Health Services (JCCHS) was established in 1975 for the multiple purposes of health guidance, prevention and control of disease in the homes and communities of Johnson County, Missouri.
JCCHS operates under County Health Center Statutes, Chapter 205, RSMo. Under its elected Board of Trustees, JCCHS serves as the County Health Center and operates three divisions: Community/Public Health Services; a federally certified and state licensed Home Health Agency and Johnson County Hospice Care, also state and federally certified.
JCCHS is located in the Innes Medical Plaza, directly behind Western Missouri Medical Center and:
- Provides client services under supervision of licensed professional nurses and physician orders.
- Promotes independence and recovery in the home setting, reducing institutional costs.
- Promotes disease prevention and discovery through education, screening and surveillance activities.
- Provides appropriate referrals for assurance of health care access.
- Works collaboratively with community groups and providers to address health care needs.
- Provides Public Health and Environmental Public Health Services for the community.
