4-904.11A Single service/self service forks were mixed in different directions in serving basket. CORRECTED

3-301.11B 1) Employee used bare hand and cup to scoop ice for drinks. 2) Employee barehanded fruit into drinks. Manager discussed getting ice scoops and holders for ice wells and tongs for bar fruit as well as speaking to employees about hands touching ice using cups for ice and bare handing fruit. CORRECTED
4-601.11a Ice bucket and ice scoop at were laid on top of ice machine in lounge area. Discussed with manager that washable/sanitizable trays may be used. CORRECTED

5-501.17 Female restroom/stalls did not have a covered receptacle for sanitary napkins. CORRECTED

4-302.14 No test kit for sanitation. Kitchen manager and bar manager said they would pick up test strips as required. CORRECTED

6-501.11 3 women toilets were very loose in their attachment to floors. CORRECTED

4-601.11c 1) Kitchen refrigerator glass door soiled. 2) Exhaust hood condensers over grill/fryers dusty. 3) Walk in cooler (now pantry) has lemons/debris on floor. Functioning walk in cooler too has debris. CORRECTED

3-302.12 Kitchen refrigerator had numerous tubs of food that were unmarked to content and did not have dates labeled. Staff did not know when prepared. Staff discarded foods. CORRECTED.
6-501.111b Kitchen ice machine had dead cockroach on top of machine lying next to ice scoop. CORRECTED.
5-205.15B Many girl bathroom toilets were loose in their mounts.
6-501.12A Main walk in cooler has mold growing on ceilings. Manager discussed using a special sealing paint to fix.

4-601.11C 1) Soda gun holsters at bars not completely clean. Staff leaving holster in place and trying to reach in unassembled holster with a rag to clean it. Corrected. 2) Shelf above deep fat fryer dirty. CORRECTED
4-601.11A Ice machine east end of building. Has black material and mold on ice deflector lid and exterior of machine. CORRECTED
JC-3-4-5-4B Ensure every manager is currently trained in food safety. 25% of food handlers on duty must be trained and hold a current food handlers permit.

Core Violations:
4-101.111 Several areas where new wood shelf counter partition has been installed and not coated painted. Noted 4/23/15 and again today. Many areas have been coated painted. Bottle rack in west bar the lower shelves are still bare wood. Shelf for small refrigerator in far west bar is bare wood. As moister condenses on this shelf it will grow mold and rot.
6-501.11 Repairs being made to west kitchen and ceiling in east end of the bar. Hole thru floor in middle bar. Appears to be just in the decking.
4-402.12A Shelf in food prep area, East side of building has a dirty floor under a shelf that is only two inches off the floor. CORRECTED

COMMENTS:
Temperature Log:  Hot dogs 39°F Coca Cola cooler bar tablet sanitizer available at all bars. Chlorine bleach available at three vat sink at west kitchen.
