4-601.11A  Ice machinie had yeast in chute seam and minor mold in upper unit within access port. Beer pitchers stored lip down on top of refrigerator with dust accumulation on top. CORRECTED

4-601.11C   Light switch string in kitchen has accumulation of dust/debris in a food prep area. CORRECTED 

4-301.11     Prep station lower unit does not maintain temperatures below 41°F.  According to staff this lower unit is only used temporarily in mornings to feed the upper prep tubs and then all contents are stored in walk in cooler overnight. The foods measured from 42-48°F and as needed are placed directly on ice in upper prep station. CORRECTED

Items in freezer are thawing but still within safe temperatures. Observed the freezer with a build-up ice on the shelving. This maybe causing the door to not close properly. The manager stated he will defrost the unit.
